This is a great opportunity to replace EL2 forms (physicals) that are about to expire and/or to get ready for the upcoming 2025-2026 school year's participation in BMC sports.
Athletic Clearances profiles for the 2025-2026 school year will be open after May 2, 2025.
In addition, new this year is that ECG/EKG test results are a mandatory requirement as part of the documents needed for your Athletic Clearance.
Answers to frequently asked questions:
- ECG/EKG tests will not be provided on Physical Night.
- Families can take their student-athletes to their personal doctor, Centra Care, Nemours or any other place that offers these services. There is an organization that has screenings all over Orange County in which ANYONE can sign up regardless if they are a student from that institution. - ECG/EKG will last 4 years, but after they have the results, it is up to each family/student to upload the test results every year in the Athletic Clearance profile.
- For sports to continue without interruption (such as Football and Cheer among others), athletes must have a current and up-to-date profile for 2024-2025 and then, on May 2nd, create a new one for the 2025-2026 school year (This is mandatory for those athletes).
- It is the responsibility of the parent/guardian/student to upload a new EL2 form when it expires. These forms expire one year from the date they were issued. Please contact your coach, an associate athletic director, or Alex Senior to be re-cleared after submitting and uploading your updated EL2 form to your athletic clearance profile.
- Creating a profile DOES NOT mean your child is cleared to play sports at BMC. Athletic Clearances will be reviewed and processed during business hours, Monday through Friday, between 7:30 am and 2:30 pm. Please file the clearance in a timely manner before tryouts/training/practices to allow for a few business days to process forms/profiles.
- There is a NEW updated 5-page form (revised 2/25) that you can print from the BMC website or the FHSAA.com page. This form WILL NOT be considered valid unless ALL sections and blank spaces are filled out correctly, contains the physician's signature and date, and have the watermark stamp of the physician's office.
- BMC requires all pages of the EL2 form to be uploaded (not just page 4 as the platform states). The old 2-page EL2 Form WILL NOT be accepted anymore.
- If you are a transfer student coming to BMC as a 10th, 11th, or 12th grader, you must complete a GA4 form EVERY YEAR.
