All incoming students who wish to participate in athletics must first submit a completed physical and FHSAA consent and liability form. These forms are valid for one calendar year from the date of the physical.
These forms must be on file with the athletic department prior to any student or prospective student being allowed to attend tryouts or participate with any athletic team. Please go to the BMCHS home page, click on "Athletics" then click on "Student Athlete Info" to download these forms.
Note: Florida Department of Health generic forms are not acceptable!
